Education for Healthy Living

Hospital patients with chronic diseases don’t have to struggle alone to regain their health.  We’re here to help with education, support and guidance to teach them how to manage their disease and prevent progression, allowing hospital staff to focus on their most critical patients.

Our trained nurses and health aides can work with each patient to create an educational plan and routine that sets them on the path to wellness.  Plans include:

  • Arthritis
  • Asthma
  • Alzheimer’s Disease
  • Cancer
  • Cardiovascular Diseases
  • Diabetes
  • Obesity
  • Stroke

The initial consultation is free.  Afterwards, our staff will work directly with the patient on their own personal wellness plan.
